The Role: Registered Nurse Case Manager
This position is approximately 70% in-person and 30% remote.
We are seeking a reliable and passionate Registered Nurse to join our home health care team as an RN Case Manager. In this role, you will develop care plans and deliver high-quality, skilled nursing to children with intellectual and developmental disabilities (IDD) in the Fort Collins, CO area.
Your Responsibilities
Conduct home visits every 60 days to assess patient needs.
Create and implement care plans, provide in-home care, and monitor patient progress.
Educate and support parents about their children's health.
Utilize telehealth tools to maximize your impact, communicate with the care team, and engage with families.
Advocate for children's health outcomes and further our mission.
Requirements
Must be a Registered Nurse with at least 2 years of experience and a current Colorado license or compact license.
Experience with and training family CNAs is preferred.
Ready to thrive in a fast-paced, start-up environment.
Must reside in the Fort Collins, CO area or be willing to relocate.
CPR certification is required.
Home care experience is essential.
Pediatric experience is preferred.
BLS certification is required; PALS certification is preferred.

Daily Schedule
Monday: Mandatory nurse meeting in Denver from 9 am to 10:30 am, followed by an admin day to complete tasks from home.
Tuesday to Friday: In-person recertifications (maximum of 3 per day, usually 2); in-person start-of-care assessments (maximum of 3 per week, usually 1 per day); remote work for charting, family connections, and case management.
