**Job Title: MDS Coordinator**

**About Us:**

We are a respected long-term care facility dedicated to providing exceptional care and support to our residents. We are currently seeking a skilled and compassionate MDS Coordinator to join our team. This position plays a vital role in ensuring the highest quality of care through effective assessment and documentation.

**Key Responsibilities:**

- Conduct comprehensive assessments and complete MDS documentation in accordance with federal and state regulations.

- Collaborate with interdisciplinary team members to develop and implement individualized care plans based on assessment findings.

- Monitor resident progress and ensure accurate coding and data entry for reimbursement purposes.

- Provide education and training to staff on MDS processes, documentation, and compliance requirements.

- Stay current on regulations and best practices related to MDS and long-term care.

- Participate in quality assurance and performance improvement initiatives.

**Qualifications:**

- Current RN or LPN license in WI

- 3+ years experience in long-term care or skilled nursing facility

- Strong knowledge of MDS regulations and processes

- Excellent organizational, communication, and interpersonal skills

- Ability to work collaboratively within a team environment
